SOUTH CAROLINA TURNING POINT LOCAL INITIATIVES

The Turning Point Local Grant Initiatives are local regions that currently conduct community health assessments. The first round of funding was divided into three branches: PRO Hampton County, Partners for a Healthy Community in Anderson County, and Community Coalition of Horry County.
The second round of funding was also divided into three branches: Edisto Health District, Five Rivers Community Development Coalition in Waccamaw Health District, and Clarendon Health District.

These local initiatives seek to evaluate community health analyses and promote the preservation of healthy lifestyles by implementing various forms of support and services. By utilizing these local grant initiatives, we are able to create and synthesize recommendations to improve the entire state’s public health system. Each initiative operates under differing strategic plans to achieve their own specified goals and objectives.

Several programs have been established under each of the local grant initiatives that have proven to help local communities gather information, maintain a healthy lifestyle, and give support to those in need.

PRO Hampton County Mission:
The mission of PRO Hampton County (PROHC) is to foster Pride, Respect, and Opportunity for all Hampton County citizens by empowering the community to work together for an improved quality of life and by opening doors of communication, collaboration, and volunteerism.

PRO Hampton County:
PROHC conducted eight community forums where community issues were assessed and prioritized. For task forces were formed: Economics, Intergroup Relations, Education, and Health and Lifestyles. The task forces felt it was time to reevaluate priorities and identify and address new community health issues.

PROHC developed the first-ever Health and Human Services Resource Directory listing agencies, organizations, and other service providers in the county. The directory, a local priority, was completed in May 2001 and was distributed as part of the Turning Point grant. At a February community forum, participants discussed major health issues and identified two priorities: health education/awareness and access to health care. Two workgroups were created to address the priorities. The workgroups meet monthly. A Health Coordinating Council resulted from the community forum and working with the CDC Local Public Health System Assessment. The council meets every other month to discuss their community roles and responsibilities and how they can work together to bridge gaps in needed services.

Partners for a Healthy Community

Partners for a Healthy Community Mission:
To improve the health and well being of people.

Partners for a Healthy Community Vision:
To develop and nurture healthy communities through partnerships that are dedicated to continuous improvement of the health and well being of people.

Partners for a Healthy Community:
When Partners examined the national Healthy People 2010’s Ten Leading Indicators, they realized they addressed all of the indicators except for the environment. Partners used NACCHO’s environmental health assessment tool, Protocol for Assessing Community Excellence in Environmental Health (PACE-EH), as a guide to conduct the assessment. Environmental status and public perception information were presented in a one-day Strategic Planning Retreat in February 2001. This process engaged additional stakeholders with key roles identified through the assessment process as critical to Anderson’s environmental future. Two workgroups, Waste Management and Smart Growth, were formed and have been meeting monthly to further discuss and prioritize their goals.

Community Coalition of Horry County

Community Coalition of Horry County Mission:
To build collaborations that maximize community assets to strengthen the health and well being of our families and neighborhoods in Horry County.

Community Coalition of Horry County Vision:
Horry County as a healthy community.

Community Coalition of Horry County:
The Community Coalition of Horry County (CCHC) is a vehicle for community engagement and mobilization on issues such as violence prevention and youth development, HIV/AIDS, substance abuse and adolescent pregnancy prevention. Community planning around these issues pointed out a need for countywide community health improvement planning. CCHC distributed the CDC Local Public Health System Assessment (LPHSA) tool to various agency heads throughout the county. Collecting LPHSA information was difficult because most providers initially perceived the core functions as the exclusive domain of the health department. The Data Collection Workgroup used the CDC tool as an opportunity to begin broadening the perspective of the community on what are health and its determinants. Organizations that went through the assessment process understood how they fit into “public health.” The Community Leadership Council continues to collect more data through community conversations.

SECOND ROUND OF FUNDING

Edisto Public Health District

Edisto Health District has created an Orangeburg County Health Improvement Coalition. This coalition has commenced implementing the MAPP Model with representatives from each community within Orangeburg County – business, nonprofit, government, faith-based organizations, and community leaders. Orangeburg County Health Improvement Coalition has held meetings that involved two areas: (1) Leading Causes of Death, and (2) Public Health Services Matrix. The Leading Causes of Death subcommittee is planning a retreat to work on the MAPP Public Health Assessment Tool and a community matrix. The Public Health Services Matrix subcommittee is working on gaps in the community regarding public health. The coalition is currently in its marketing phase.

Georgetown

Five Rivers Community Development Coalition (CDC) heads the Turning Point initiative for the Waccamaw Public Health District in Georgetown County. Five Rivers CDC has generated a new Turning Point Strategic Plan for Waccamaw Health District. The new Strategic Plan was developed by Five Rivers Community Development Coalition and the Community Development Director for DHEC. They are now in the process of reengaging their community.

Clarendon Public Health District

Clarendon Public Health District has initiated Clarendon County Turning Point. This coalition has held a MAPP Committee meeting which identified groups to collaborate with for the MAPP process, created a public health resource list, and will actively recruit people for their next meeting. Clarendon Public health District has hired a full-time employee to assist with the Turning Point initiative in their area. Officials have also met with the County Council in order to launch Turning Point initiatives in the Clarendon County Strategic Plan.
