Thank you for your interest in the 2012 Physical Activity and Public Health Courses! The deadline for applications is May 15, 2012. You will be requested to submit:
- A sponsor letter.
- A one or two page statement indicating basis of interest in the course.
- A curriculum vitae (a resume is acceptable for the Practitioner Course). Please limit your CV to no more than 5 pages to include: educational history, professional appointments, other research training, publications, and grants.
You will be required to create an account in order to upload the above materials and will be able to use this account to check on the status of your application. Additional information regarding travel, lodging, and course fee payment will be sent to you upon your acceptance into the course.
