PROJECT OVERVIEW/
ABSTRACT:
 • A three-year study funded by the Agency on Aging and National Council on Aging

• Major goal is to implement two evidenced based health-related programs in multiple communities in several counties in South Carolina

• The two programs are the Chronic Disease Self-Management Program and A Matter of Balance (fear of falling program)

• Key components include training Master Trainers, training local personnel, implementing and evaluation of outcomes of each of the community-based programs
