If so, The FACTs Program is just what you need!!!

The FACTs Program (Functional Abilities Counseling and Testing) is designed to help older adults take some simple steps to promote functional health and prevent disease and decline.  FACTs was established in 1998 in response to the: 

  • Increasing number of older adults experiencing dramatic declines in functional abilities (loss of strength, poor balance, and/or mobility, changes in cognitive function, etc.)!
  • Increase in number of older adults heavily dependent on assistance from others to carry out daily activities!
  • Awareness that 30-40% of older adults fall each year and are hospitalized
  • Recognition that many who fall suffer fractures, enter nursing homes and never fully recover!
  • Concern that a majority of adults age 70+ have difficulty with one or more activities of daily living!
